A very nice rep at Amazon spent a long time helping me find the solution.  The fix was to update the following in Adobe Flash Player's global settings to allow 100KB of storage:&#xD;

Then close your browser.  Reopen and stream an Amazon video for at least a couple of minutes so that an average connection speed can be selected by Amazon.&#xD;

All your points are definitely things we need to work on, and I think number 4 is especially important. It's great to see that development has accelerated since our move to github, and I'm particularly pleased that the tests are now a plugin, being extended to include acceptance tests, and will let plugins and themes include tests.&#xD;

But I digress.
